(1) I am aware that this agreement for TELEPHONE SERVICE CALL OR DOWNLOAD is between Cencom Inc, hereafter called "Contractor" and Internet Subscriber, hereafter called "Subscriber".(2)Subscriber agrees to the following terms: $1.00 per minute, minimum $15.00 per phone call, minimum $30.00 per service issue.(3) A $30.00 minimum fee shall be charged to subscribers credit card on file with their account at time of this order and is non refundable regardless of reason.(4) If Subscriber does not have a credit card on file with contractor, they agree to provide such for both this service call, as well as future monitoring and repair fees from contractor.(5) Any additional fees to be charged to Subscribers account upon completion of telephone service call.(6) Any parts fees shall be charged to Subscriber in addition, plus any shipping fees.(7)Contractor is not an insurer and does not provide the Subscriber insurance or any guarantee or warranty for any work performed or failure to perform on this order by said local provider.(8) Subscriber agrees to remain a monitored account with contractor a minimum of 12 months beyond any service call performed through this order.(9) Said service or alterations provided to Subscriber by local party shall be considered an addition to the original agreement and shall be incorporated into that document, and shall be subject to the same terms and conditions as fully set forth here. (10) If any promotional codes are used for reduced fees, Subscriber agrees to a minimum 24 month term for service beyond the order form submission date.
